Hospice Philosophy
Hospice care centers on an individualized plan of physical, emotional, spiritual and administrative care for patients with a life-limiting illness.Since dying is a natural process, our professionals are committed to enhancing every patient’s quality of life through this unique personal event. The hospice program is dedicated to excellence in end-of-life care with emphasis on comfort and dignity for the dying, making it possible for them to function independently as long as possible. A hospice care team with involved with concerned family members, meaning that the hospice team respects and responds to the unique preferences lifestyles, values and wishes of each individual’s family.
Hospice is for all age groups and all diagnoses. Federal regulations require that patients meet certain criteria in order to be eligible for hospice services.
Admission criteria include:
- A diagnosis of a life limiting illness where comfort care is chosen as opposed to curative care.
- Certification by your doctor and our Hospice Medical Director that you have a life limiting illness with a prognosis of six months or less if the disease runs its normal course.
- Place of residence within our service area.
